Under FHSAA rules and Florida law, every student-athlete must complete an annual pre-participation physical evaluation before beginning practices, tryouts, workouts, conditioning, etc..
In advance of your appointment, these documents are available for you to review and complete. High school athletes who have scheduled a physical should consult with their parents while completing the EL2 form.
